Payments
Payment before your visit is greatly appreciated. We accept payments by cash, check, or credit card. There is a minimum payment of $25 on a credit card. We cannot bill your co-pay. This must be paid before leaving the office or there will be a $40 administrative charge.
Please double check with your insurance carrier to verify that we are a participating provider with your plan. If you are seen by a practitioner in our office and we do not accept your insurance, you are responsible for your balance. It is your responsibility to know about your insurance.
Monthly Billing Statements
If you have an outstanding balance and you have not paid us in a timely manner, you will receive a billing statement. Please pay this balance promptly. When mailing in your payment, please write your account number in the “memo” section of your check. By using this account number, we can be sure to credit you for the payment (and not someone else with a similar name). If you wish your payment to be applied towards a specific visit, simply indicate the date of service next to your account number on the check. If no date of service is noted, we will apply your payment to the oldest outstanding fee first.
